In-Network Insurance Providers

We are in-network and file insurance claims for the following providers:

Know Your Benefits

Your health insurance policy is a contract between you and your insurance company (or your employer).

It is your responsibility to understand:

• Deductibles
• Copayments
• Coinsurance
• Annual visit limits
• Prior authorization requirements

We are happy to provide procedure and diagnosis codes to help clarify your coverage.

For Patients With In-Network Insurance

• Proof of insurance is required before your first appointment.
• Co-pays and deductibles are due at the time of service.
• We submit claims on your behalf, but payment is not guaranteed.
• Denied claims will be billed at the contracted rate.

• Claims unpaid after 60 days may be billed to you directly.

Out-of-Network Insurance

• We do not file claims for out-of-network insurance
• You are responsible for checking your benefits and submitting our invoices to your insurer.
• We will provide detailed receipts with diagnosis and treatment codes.

Good Faith Estimate

Uninsured or self-pay patients will receive a Good Faith Estimate in compliance with the No Surprises Act.

Payment Options

• Fees are due at the time of service or end of each week.
• Accepted payment methods: Cash, Check, Credit Card
• Rates are reviewed regularly and may be adjusted at our discretion.
